The Operation Care Program provides case management and care coordination services to persons who excessively use the emergency 911 system for non-emergent and/or poorly managed health conditions. Case Managers assess the circumstances of each frequent 911 caller and provide local resources for education, system navigation, care coordination, health care linkages for the uninsured and underinsured and other referrals as needed.
